Fall 2020 Lottery Enrollment Appointments

ENROLLMENT INSTRUCTIONS*

*These instructions are ONLY for students that

have received an acceptance letter from GHC.

STEP 1:  See full instructions on this page.

STEP 2:  Fill out the online GHC enrollment form at www.ghctk12.com/enroll/.

STEP 3:  Electronically upload the documents listed below as instructed in step 1.

 

HIGH SCHOOL:

  •      Parent/Guardian driver’s license
  •      Student’s birth certificate or passport
  •      Student’s immunization records
  •      Fall semester report card for 8th grade students (received in Dec) or
  •      Completed contact sheet (enclosed with letter of acceptance)
  •      If applicable, the most recent 504 plan or IEP

TK – 8:

  •      Student’s most current immunization records
  •      Student’s original birth certificate or passport
  •      Completed contact sheet (enclosed with letter of acceptance)
  •      Parent/Guardian driver’s license
  •      If applicable, the student’s most recent 504 Plan or IEP
  •      Copy of Student’s most recent report card (voluntary)
